Primary Care to offer sports checkups for students
Primary Care’s division of Community Pediatric and Adolescent Medicine will be hosting two clinics for High School Student Athletes age 12–18 entering grades 7–12, who need a physical to participate in school sports. Students who have had a well-visit examination within the past year may not require another exam. Please speak with your primary care clinician to confirm.
Key information
- Date of Clinic 1: July 21, 2026, morning.
- Date of Clinic 2: July 30, 2026, afternoon.
- Location: Baldwin Building, third floor, 221 Fourth Avenue SW, Rochester, Minnesota.
- Who may attend: Adolescents who are current patients of Primary Care in Rochester and Kasson and their parents or guardians.
- What is not covered: No college sports physicals or camp exams will be conducted.
Requirements to attend
- Appointments are required. Scheduling for these clinics will begin May 6, 2026.
- How to schedule: Parents and guardians can directly schedule through the patient portal using the proxy setup or call the Community Pediatric and Adolescent Medicine appointment line.
- One parent or guardian accompanies the student athlete.
- Arrive no more than five minutes before the student's check-in time.
- Students should wear athletic apparel that makes it easy for an examination.
- Students should bring prescription eyewear to the appointment.
- Complete these sections of the 2026–2027 Minnesota State High School League Sports Qualifying Physical Examination Medical Eligibility form: page 2, the top of page 3 and page 4 (if applicable).
- Bring this form with you to the appointment.
Important reminders: Arrival and parking
- Construction on our downtown campus may affect your arrival and departure. Parents and guardians should check the Rochester construction updates website the day before and day of your appointment for real-time updates, to be sure you have enough time for arrival and parking.
What to expect on the day of each clinic
- During the visit, the patient will move through a series of stations to complete vision/hearing screenings, immunizations and a physical examination.
- This meets all requirements of a well-visit examination.
- Although a parent or guardian is required to accompany each student and will be present for initial portion of the visit when meeting with nursing and other members of the team, there will be times during the visit that the student will be separated when meeting with the medical professional.
