Primary Care to offer sports checkups for students

Community Pediatric and Adolescent Medicine has scheduled clinics for student-athletes ages 12–18 and entering grades 7–12 who need a physical to participate in school sports. Patients who have had a well-visit examination within the past year may not require another exam. Contact your primary care clinician with questions.

The clinics will be held on the afternoon of July 24 and the morning of August 4. The location for both clinics will be the third floor of the Baldwin Building, 221 Fourth Avenue SW, in Rochester, Minnesota.

These clinics will be available to adolescent patients who receive their primary care at Mayo Clinic in Rochester or Mayo Family Clinic Kasson. No college sports physicals or camp exams will be conducted.

During the visit, the patient will move through a series of stations to complete vision/hearing screenings, immunizations and a physical examination, which meets all requirements of a well-visit examination.

Appointments required

Appointments are required for these clinics and may be scheduled starting May 12.

Other requirements

  • One parent or guardian will be required to accompany the patient. Note that parent(s) or guardian(s) will be separated from patients during parts of the visit.
  • Arrive no more than five minutes before the patient's check-in time.
  • Wear athletic apparel that allows for an examination.
  • Bring prescription eyewear with you to the appointment.
  • Complete page 2, the top of page 3 and page 4 (if applicable) of the 2025-2026 Minnesota State High School League Sports Qualifying Physical Examination Medical Eligibility form and bring these pages with you to the appointment.
