Primary Care to offer sports checkups for students
Community Pediatric and Adolescent Medicine has scheduled clinics for student athletes ages 12–18 and entering grades 7–12 who need a physical to participate in sports.
These clinics will be held July 25 and Aug. 2. The location for both clinics will be the third floor of the Baldwin Building, 221 Fourth Avenue SW, in Rochester.
The clinics are available to adolescent patients who receive their primary care at Primary Care in Rochester and Kasson. No college sports physicals or camp exams will be conducted.
During the visit, the patient will move through a series of stations to complete vision/hearing screenings, immunizations and a physical examination, which meets all requirements of a well-child examination. Patients who have had a well-visit examination within the past two years may not require another exam. Contact your primary care provider with questions.
Appointments are required for these clinics. Appointment phone lines will open April 25. Contact the patient's primary care provider's appointment office.
Direct schedule will also be available through Patient Online Services, beginning April 25. Patients 13 to 18 need to have their own account to schedule the visit. For children 12 years of age, the parent or guardian can schedule using the proxy set up.
Other clinic requirements
- One parent or guardian will be required to accompany the patient. Mayo Clinic's visitor policy will be followed during these clinics. Note that parents or guardians will be separated from patients during parts of this visit.
- Social distancing and masks will be required by the patient and the parent or guardian.
- Arrive no more than five minutes prior to the patient's check-in time.
- Bring the 2022-2023 Minnesota State High School League Sports Qualifying Physical Examination Medical Eligibility form. Complete Page 2 and the top of Page 3, as well as Page 4 if applicable, prior to arriving at the clinic.
