The Senior Services program provides care to older adults with complex medical needs in their home or skilled nursing facility.
This program provides four types of services. The Care Transitions Program is designed to help patients during the critical point of transition from the inpatient to outpatient setting. The Palliative Care Homebound Program is a home-visit services provided to individuals with late-stage, life-threatening illnesses. Primary Care House Calls is designed to help patients with two or more activity-of-daily-living needs by providing home visits, creating care plans, reviewing medications and more. The Skilled Nursing Facility staff provide medical care/consulting services to Rochester and surrounding community skilled nursing facilities, working in partnership with their staff to create a health care team to serve the needs of the residents.

Community Internal Medicine
Community Internal Medicine cares for patients age 18 and older. Our providers offer general health and wellness care, including preventive exams. They also help manage complex medical conditions, such as heart disease, emphysema and diabetes.
