Care Transitions Program / Palliative Care Homebound Program
The Care Transitions Program is designed to assist frail elders (age 60 and greater) during the critical point of transition from the inpatient to outpatient setting. The Palliative Care Homebound Program is a home-visit service provided to homebound, high-risk, frail individuals with late-state, life-threatening illnesses.

Family Medicine staff care for people of all ages, providing family oriented comprehensive care, including obstetrical care and well-woman care, newborn and well-child care, pediatrics and adult/geriatric care.
