School flu online sign up opens August 7

It's time to sign up to have your child vaccinated at school! The deadline for signing up is 11 p.m., Wednesday, September 11. No late online or paper forms will be accepted. 

The School-Located Immunization Program (SLIP) provides influenza (flu) vaccines at the schools each year. It's supported by Mayo Clinic, Olmsted Medical Center, Olmsted County Public Health Services and partners with Dodge County Public Health Services, most Olmsted County public and private schools and Dodge County Kasson-Mantorville public schools. 

There is one shared sign-up period for 2019: 

  • Sign up is ONLY open August 7 through September 11 at 11 p.m.
  • Late sign ups aren't allowed â€” NO EXCEPTIONS.
  • Online sign up is preferred. it's quick and easier than ever, especially for those with two or more children. 
  • Paper forms will be available for those who need them at Meet the Teacher events and in the school office. 

The injectable (shot) and nasal spray (FluMist®) vaccines will be available. For those choosing the shot, non-medicine, pain-reducing options will be offered. Teachers will also have a short video for their elementary classrooms to ease their students' concerns. 

 

We've learned there will be a shortage of the nasal spray flu vaccine this year. This means it may not be available at your children's school. If you choose your children's vaccine choice as "nasal spray only", and we don't have it that day, your children won't be vaccinated at school.
