Kasson offers free sports checkups for students
On Wednesday, June 19, students who need a physical to participate in sports can receive a free check-up at Mayo Family Clinic Kasson. This sports physical clinic is open to any Dodge County student entering grades seven through 12 in the Kasson-Mantorville, Hayfield and Triton school districts. No appointments are necessary for this free clinic.
Only sports screening exams will be conducted; as in the past, students will visit several examination stations. No immunizations or treatments will be provided. If a medical concern is discovered, the student will be given instructions for follow-up care.
When: Wednesday, June 19, 4:30-6 p.m. Students must be at the clinic no later than 6 p.m.
Where: Mayo Family Clinic Kasson, 411 West Main, Kasson, Minnesota
What to wear: Students should dress casually with shorts and t-shirt (weather permitting)
What to bring:
- Mayo Clinic identification number, if they are a current patient
- Minnesota State High School League Annual Sports Health Questionnaire (Form should be completed before arriving at the clinic.)
- Minnesota State High School League Sports Qualifying Physical Examination Clearance Form
