Primary Care Sports Clinic
The sports clinic will be open to student athletes ages 12–18 and entering grades 7–12 who need a physical to participate in sports. No college sports physicals or camp exams will be conducted.
During the visit, the patient will move through a series of stations to complete vision/hearing screenings, immunizations and a physical examination, which meets all requirements of a well-visit examination.
Patients who have had a well-visit examination within the year may not require another exam. Contact your primary care provider with questions.
Cost
This examination meets all requirements of a well-visit examination. The patient's insurance will be billed for the visit.
Requirements / Who Qualifies
- Students ages 12–18 who are entering grades 7–12 who receive their primary care at Primary Care in Rochester and Kasson.
- Arrive no more than five minutes prior to the patient's check-in time.
- Bring the 2023-2024 Minnesota State High School League Sports Qualifying Physical Examination Medical Eligibility form. Complete Page 2 and the top of Page 3, as well as Page 4 if applicable, prior to arriving at the clinic.

Appointments are required
- Students ages 12–18 who are entering grades 7–12 who receive their primary care at Primary Care in Rochester and Kasson.
- Appointment phone lines will open May 1. Contact the patient's primary care provider's appointment office to schedule.
- Direct scheduling will also be available through Patient Online Services, beginning May 1. Patients 13 to 18 need to have their own account to schedule the visit. For children 12 years of age, the parent or guardian can schedule using the proxy set up.
